2024-2025 fIRST kIDS cHRISTIAN pRESCHOOL rEGISTRATION

Please complete this form and submit a non-refundable registration fee of $60.00 to hold your child’s space for the school year. Registration forms and fees can be submitted online, by mail or in person. Note: A $3 processing fee is charged for online payments.

Please fill out one form for each child.

2024-2025 Enrollment Packet

These forms should be filled out (completely with no sections left blank) and turned in by our Open House on August 25. You can drop off completed paperwork to us at 110 W. Franklin St. in Troy or email it to preschool@troyfumc.org.

Enrollment Packet

Required mEDICAL Forms

Each student must have a signed medical statement and shot record on file. Please schedule your child's well check in time to have this form completed by the doctor before schools starts. Please refer to the Welcome Packet you received for all of the required paperwork or download a copy of the form here.
Medical Statement
If your child has any specific health concerns, please let us know. A separate form will need to be signed by the doctor and kept on file for each condition. We do not give any over the counter medicines.
